BILLING AND FEE FAQ

Washington Medicaid Patients

AAC will bill Medicaid directly for your services. No payment will be expected from you unless you do not have active coverage.

Oregon Medicaid Patients

AAC participates with some, but not all, Oregon Health Plan products and Oregon Community Care Organizations (CCOs). Please contact our office first to see if we participate in your plan.

Commercial Insurance Patients

AAC is not contracted with commercial insurance companies. AAC charges a set fee to our patients and payment is due prior to the date of service. Any cancellation or reschedule made less than 24 hours before an appointment will result in a cancellation fee of $300. If you have not received an email from us explaining our fees, please call our office. Please note that our fee is the maximum amount you will be responsible for.

We are out-of-network with all commercial insurance companies and do not submit claims to the following companies: TRICARE, BCBS FEDERAL PLANS, KAISER AND PROVIDENCE.

As a courtesy, we will submit a claim to your medical insurance carrier on your behalf. The total claim amount will be based on the total minutes of anesthesia done. If we collect and there is an overpayment from the TOTAL due we will process a partial refund in the amount of the overage to your account. The refund, if any, will be processed in the form it was received. If we do not collect from your insurance, we will not bill you for the remainder due on the account.
 
Our office does not have any specific information regarding pre-authorizations, exclusions, limitations, or waiting periods that may be required by your insurance. If you have any questions, please contact your insurance company directly.
 

We bill medical procedure code 00170

We make NO guarantee of what your insurance company will pay; this is a contracted benefit between you and your insurance company. Claims require 2-4 months to process.
 

Cash Pay Patients
